This policy applies only to information collected through the Site. It does not cover the protected health information of patients who receive care from CBH Medical in correctional facilities. That information is handled under the Health Insurance Portability and Accountability Act (HIPAA), other applicable laws and our agreements with the facilities we serve.

- Information you give us. When you use our contact form or email us, we collect the information you choose to provide, such as your name, organization, email address, phone number, the topic of your inquiry and your message.

We use this information to operate and secure the Site.
- Cookies. The Site does not use advertising or cross-site tracking cookies. Some content, such as fonts and map data, is delivered by third-party services such as Google Fonts and Cloudflare, which receive your IP address in order to deliver that content to your browser.
- Job applications. Our open positions are posted on Indeed. Applications you submit there are collected by Indeed under its own privacy policy.
Please do not send health information
The Site is not intended to collect health information. Please do not include medical details or information about a patient’s care in a contact form or email. If you have a question about a patient’s care, please call us at 215-542-1010.
